The first step in applying for admission to Phillips-Strickland House is to complete an application form.
Download Phillips-Strickland Admission Form (pdf)
Mail the completed form to :
Janet Kimball
Director of Social Services
Phillips-Strickland House
21 Boyd Street, Bangor ME 04401.
Once we have the completed form, you will be considered for admission. When we have an opening, we will contact people on the waiting list.
If the time is right, we will schedule a tour and an interview. At that time we will ask that you sign a release to allow us to gather medical information from your primary care physician.
Once all the information is complete, it will be reviewed by the admissions committee. Their findings will be passed on to the family as soon as possible.
If you have specific questions, please contact Janet directly at jkimball@pshouse.org.
We are currently accepting applications to Boyd Place.
Applications are placed on a waiting list according to the floor plan of choice. Once there is an opening, you will be called and given the option of arranging for an interview or staying on the waiting list.
Family members can arrange the appointment for an interview, but the prospective resident will be required to be present for the interview.
Boyd Place is for independent seniors and the interview process is required to determine if Boyd Place can meet the needs of you or your loved one.
If you have questions about this process, don't hesitate to contact Sharon Nickerson at snickerson@pshouse.org or 207.941.2820
